Online Medical Cards

We have Online Medical Marijuana Card appointments available throughout the week - including SAME DAY APPOINTMENTS. Give us a call to schedule your appointment or easily SCHEDULE ONLINE.

Our Online Visits consist of a Video Chat with one of our marijuana doctors done from the comfort of your own home. You can easily visit us using either a laptop or mobile device. We then help you submit your application for Arizona to process and provide you with a digital medical marijuana card that you are able to use throughout Arizona and any other states that accept it.

Green Leaf takes pride in being the safest Medical Marijuana Doctors in Arizona. We have instituted many safety precautions for our patients and team during these uncertain times. If you prefer to avoid the office experience, these Video Chat appointments allow you the convenience and comfort of your own home or office.

Patients Without Medical Records

For patients who do not have valid medical records from within the last 12 months, we do have doctors who can perform physical exams to diagnose some medical conditions. Online, these exams are limited to upper body muscular or skeletal pain ailments that have been chronic (6 months or longer).

Please give us a call with any questions.

Medical Records Information

We accept medical records showing that you have been treated for a Qualifying Condition within the past 12 months. These summaries must include the diagnosis about the qualifying condition(s). We do not accept discs or pictures as medical records. We must have your records at the time of your appointment to be able to use them.

Here are some common ways patients obtain their medical records:

1. Visit your doctor's patient portal and download a recent visit summary that includes your diagnosis.

2. Call your medical provider's office and ask for the chart notes from your recent visit(s). (Some offices will charge for this)

3. Fill out our Release of Records form, and we can fax it to your medical provider for you if you send it back to us (make sure it is signed and the fax number is correct).

If you choose option 3, your doctor may take a couple of day and up to 4 weeks to send us the records.
